(1) Children age 17 and under MUST have a Signed Consent by a Parent or Legal Guardian on or before their 1st visit.

(2) If you are unable to bring your child to their visit, you MUST sign a parental consent form allowing a SPECIFIC family member or friend to accompany your child.

Who will my child see during their visit?

When you arrive for your child's cleaning and exam appointment, you will meet one of our four doctors; Dr. Evan, Dr. Hanh, Dr. Leslie, or Dr. Xuemei. The doctor that you will see at your exam appointment will be the first available doctor, to shorten your waiting time.

If your child needs to schedule a treatment appointment, you will have the option of selecting the doctor of your choice.

Please let one of our friendly staff members know if you have any questions and we will do our best to meet your needs.
